    ERDC Special Needs Rate Form

    Form DHS 7486 is a form that assists parents is applying for the ERDC Special Needs Rate. As a provider, you can keep several copies of this form handy to give to families who are applying or are already elligible for ERDC. In fact, we recommend that you include a copy of this form with the materials you hand out when parents tour your program -- have a discussion with parents when they tour your program. You could also include this form with your enrollment packet. 

    We recommend parents use this form and get a copy of it date/time stamped when handing it in at their local DHS office so that you and the parent can track when the request was made. 

    Our Union leadership is hearing that a high number of parents (and their providers) are being denied application for the special needs rate (most of the time what we hear is that the DHS worker claims the parent failed to request the rate and for whatever reason the DHS eligibility worker failed to ask the family about the special needs while sometimes the worker is refusing to process the special needs rate and refuses the parent request).
